We offer pregnancy verifications based on the date of your last menstrual period (LMP).  Some doctors or clinics may require this.  The Pregnancy Verification process follows the same format as a pregnancy test visit and requires a Medical Services Intake Questionnaire and a clinic-administered pregnancy test. If your test reads “positive”, the nurse will give you a pregnancy verification form.  Sometimes Medi-Cal or WIC requires a verification as well.   

If we have scheduled an ultrasound appointment (a staff nurse must approve all ultrasound appointments) the nurse can do a verification based off of the ultrasound.
